Notice of Failure to Provide Essential Services

To: [Larry Landlord

Lincoln, NE 68508]

From: [Terry and Theresa Tenant
123 Tenant Trail
Lincoln, NE 68508]

I am writing to notify you that you have failed to provide the following essential services: [hot water - the water heater burst] at the rental property located at: [123 Tenant Trail, Lincoln, NE 68508] since [4/25/2015]. Because of this: (check one)

[__]I/we will purchase the service and deduct the cost of doing so from rent;

OR

[ X ]I/we will be residing at [456 Parental Parkway, Lincoln, NE 68508] until you have notified me/us that the services have been restored. I/we will be excused from paying rent until the services have been restored and you have notified me/us of this.
